Instructions for Completing the MSU-IACUC Annual Update/Amendment Form
Forms, instructions, policies, and technical information regarding approval to conduct projects using vertebrate animals are available through the MSU Regulatory Compliance web site http://www.orc.msstate.edu.
This short form should be completed when you receive notice of an annual update due from the Regulatory Compliance Office. This action ensures that the protocol(s) approved reflects accurately what you are doing with animals. This form should also be completed to request IACUC approval to change to the initial project in some way.
Enter the title, primary investigator(s), participating personnel, and original protocol number.
1. Check one or more of the reasons for submitting this form. Note that extensions are granted for a minimum for 6 months even if you believe the work will be completed in a shorter time period.
2. This section asks if you have changed anything, and, if you have, in which ways: personnel (note emphasis on training); procedures; pain relieving or euthanasia agents; numbers of animals (refer to original protocol) and pain category; sponsor; termination date; and any other changes. Requested changes in number of animals must be justified. You may under no circumstances except by the IACUC-approved amendment process use additional animals in a study or class.
It is important to remember that the IACUC may require that an entire protocol be rewritten if a change is "major" or numerous.
